    - comments: 2-102. 14(o) observed no employee health policy on site. Instructed to provide an employee health policy that includes a signed acknowledgement from each employee. Priority foundation 7-38-010.

    5. Procedures for responding to vomiting and diarrheal events
    - comments: 2-501. 11 observed no written procedure nor adequate supplies on site to properly respond to vomit and diarrhea events that can potentially occur inside of the facility. Instructed to provide. Priority foundation 7-38-005.

    10. Adequate handwashing sinks properly supplied and accessible
    - comments: 5-203. 11 observed no hand washing sink located at the front customer service counter where nachos with meat and cheese are being served. Instructed to install a hand washing sink in said food prep area and maintain. Priority foundation. Citation issued 7-38-030(c).

    - comments: 4-602. 11(b-e) observed 3 unit crock pot bowls need to be cleaned and sanitized. Instructed to clean and sanitize all cookware and utensils.

    - comments: 4-302. 12 observed no metal stem thermometer on site to take temperatures of cold and hot foods. Instructed to provide. Priority foundation 7-38-005.

    - comments: 6-501. 111 observed evidence of pest activity. Observed about 40 rodent droppings located on the lower shelves thru out the facility, about 40 rodent droppings located in the rear by the 3 compartment sink and about 30 rodent droppings located in the utility closet. Instructed to clean and sanitize the affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

    39. Contamination prevented during food preparation, storage & display
    - comments: 3-305. 11 observed foods being prepped in a electric stove top in the rear of the facility on top of the 3 compartment sink with barriers or protection from the premises. Instructed to provide an actual food prep area before any foods are prepped in the rear of the facility.

    48. Warewashing facilities: installed, maintained & used; test strips
    - comments: 4-302. 14 observed no chemical test strips on site to properly test the sanitizing compartment of the 3 compartment sink. Instructed to provide the proper chemical test strips for utensil sanitation. Priority foundation. Citation issued 7-38-005.

    - comments: 4-601. 11(c) observed all non food contact surface with accumulated debris. Instructed to clean and sanitize all.

    - comments: 6-201. 11 observed broken and stained ceiling tiles thru out the facility. Instructed to replace all broken and all stained ceiling tiles.
