- comments: observed no certified food manager on duty nor a certified food managers certificate posted to view. Must provide and maintain. (priority foundation 7-38-012)
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing sink in prep/dish washing area with hot and cold running water. Must install a hand sink. Instructed to use adjacent sink for hand washing. (priority foundation 7-38-030(c)

- comments: observed no date markings on tcs ready to eat foods being held for more than 24 hours inside of walk in cooler. Must provide and maintain. (priority foundation 7-38-005)
36. Thermometers provided & accurate
- comments: observed no probe thermometer for taking temperatures of food. Must provide and maintain. (priority foundation 7-38-005)
38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 60 or more mice droppings on floor in toilet room,behind service counter storage area along ledges and shelving units,under 3- compartment sink,rear storage area under storage shelves and sales floor. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 4/30/20. (priority foundation 7-38-020(a)
38. Insects, rodents, & animals not present
- comments: observed an appx. '1/2-3/4' gap along top of front door and rear door in center and around lower door hinge. Must make tight fitting.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must repair or replace deli cooler door in poor repair.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no chemical test kit to check sanitizing solution ppm's at 3- compartment sink. Must provide and maintain. (priority foundation 7-38-005)
49. Non-food/food contact surfaces clean
- comments: must clean debris build up on meat slicer,walk in cooler -fan guard covers,pop sleeves,display racks and shelves on sales floor.
55. Physical facilities installed, maintained & clean
- comments: must repair or replace stained and damaged ceiling tiles & floor tiles,openings around pipes and wires leading into ceilings where needed,hole in wall in rear storage area at service counter and above 3- compartment sink. Damaged wall bases & missing wall outlet cover.
57. All food employees have food handler training
- comments: observed no food handlers training for employees. Must provide and maintain.
