16. Food-contact surfaces: cleaned & sanitized
- comments: ice machine not maintained. Interior of ice machine water reservoir with excess pink and black slime substance, also interior of ice bin. Ice is used for human consumption. Instructed to clean, sanitize and maintain unit. Priority foundation violation:7-38-005, citation issued.

38. Insects, rodents, & animals not present
- comments: we observed over 30 live (adults and babies) roaches in different area of the kitchen: at rubber gasket inside walk-in cooler and reach-in cooler, can opener, electrical boxes, under duct tapes around the insulation of the walk-in cooler door, under low temp dish machine etc. Roaches also on wall cracks and crevices. Instructed to contact pest control. Must eliminate roaches clean and sanitize area. Priority foundation violation:7-38-020(a), citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: waste pipe above take out food containers in storage area must be encased or relocate items. . Surface must be smooth cleanable and non-absorbent surface

- comments: inverted pots stored on shelf too low to the floor,must store mention items 6' above the floor or relocate pots.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: walk-in cooler door in poor repair,repair with tape and foam not completely sealed. Interior of the same door is curved surface not smooth cleanable. Repair properly said door or replace. Also broken electrical boxes throughout the premises must be replaced

- comments: burned food at interior of stoves,grease build-up inside the cabinet of deep fryers, must be clean and maintain

51. Plumbing installed; proper backflow devices
- comments: pipe under three compartment sink at bar area is leaking,also water line behind and under three compartment sink in the kitchen,instructed to repair

55. Physical facilities installed, maintained & clean
- comments: broken walls under the bar counter top,unsealed wall panels in the kitchen specially under the dish machine. Must repair and seal said walls. Surface must be smooth,cleanable and non-absorbent surface. Missing wall tiles inside the women's washroom(east wall)above the handwashing sink. Removed grout from floor by the low temp dish machine and missing floor tiles(north-east corner) at same area,instructed to re-grout floor and provide floor tiles. Metal sheet on floor inside the walk-in floor broken reseal/replace

55. Physical facilities installed, maintained & clean
- comments: excess debris and grimes on floor under bar counter top,under crates. Also under the booths. Clean and maintain.
