- comments: outer openings not protected. Observed rear exit door not rodent proof. Door has 1/2 inch opening on bottom of the door. Must make door tight fitting to prevent pest entry.

- comments: observed slow draining at the front exposed handwashing sink in the front food preparation area. Must repair and maintain.

- comments: observed excessive dust accumulation on walls ventilation vent in sitting area. Must clean and maintain. Observed dirt and debris on floor in rear hallway. Must clean and maintain.
