- comments: observed evidence of appx. 8-10 live small flies around floor drain and in rear prep area,3-5 large flies flying around premises. Appx. 40 or more mice droppings on ledge at front window,under shelving units ,under ice machine in rear food and storage area and on shelf behind laundry detergent. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 6/8/20. (priority foundation 7-38-020(a)

- comments: observed an appx. '3/4-1' gap along bottom of front door. Must make door tight fitting.

- comments: must clean food debris build up from grill table,prep coolers,prep table,meat slicer,mop sink basin. Must defrost ice cream freezer.

- comments: must repair or replace leaky middle pipe at 3- comp sink.
