    - comments: manager provided no proof of employee health policy on site. Instructed to provide an employee health policy that includes a signed acknowledgement from each employee. Priority foundation. 7. 38. 010. Citation issued

    - comments: observed that employee wore rings and bracelets on hands while working with food. Must remove all jewelry except plain wedding band from hands while working with exposed food

    - comments: observed torn rubber gasket on door to reach in freezer in rear storage area and walk in cooler. Instructed to repair and/or replace and maintain

Instructed not to use crates for elevation to prevent hiding places for pest. Must provide appropriate shelving for storage. All food items must also be stored 6" away from the wall

    - comments: previously cited priority foundation violations from 6/30 2020 report # 2382447 observed not to be corrected as follows: violation #10 no hand sink in the front serving area. Instructed to install with hot and cold running water under city pressure and provide paper towel and hand soap. Also observed that previously cited core violation #60 from 8/12/2019 report #2304523 observed not to be corrected as follows. Violation # 39 instructed to install a splash guard on the left side of the exposed hand sink in the chicken prep area and hand sink located close to the drainboard for clean dishes in the dish washing area. Violation # 57 no proofed of food handler training certificate for some of the employees handling food. Must provide. Violation upgraded to priority violation #7-42-090 citation issued
