Mushroom Pad Thai Illness

On June 2nd If a mushroom is in bad shape I ordered a pad thai, coconut curry. The mushroom was very black and when chewed on it it felt very slimy and too intense in taste. At 5 o'clock I was dying with a fever of 39 degrees.  | Symptoms: Diarrhea, Nausea, Fever, Vomiting
