- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

- comments: observed a temperature of 79. 3f at hand sinks in dish washing area & toilet rooms and 3- compartment sink. No hot running water on premises at fixtures. Must maintain at a minimum of 100. 0f. Must repair and maintain. (priority 7-38-030(c)

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 75 or more mice droppings on floor behind crates near electrical panel,under and in between articles and shelving units in rear storage area. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 5/21/20. (priority foundation 7-38-020(a)

- comments: observed an appx. '1/2-3/4' gap along bottom of front and rear doors. Must make doors tight fitting.

- comments: observed a brown dog on a leash being accompanied by a male customer as he walked around the store shopping and then approached register and picked the dog up and held him in his arms as he paid for items and the employee never said a word to customer about the dog being in the store. Customer left store. Instructed manager that service dogs are only allowed on premises. (cos)(priority foundation 7-40-060)

- comments: must provide sink stoppers for 3- compartment sink.

- comments: observed no chemical test kit to check sanitizing solution ppm's for multiuse utensils at 3- compartment sink. Must provide and maintain. (priority foundation 7-38-005)

- comments: must clean debris build up from window sills,walk in cooler -fan guard covers,shelving units and sleeves. Must defrost ice cream freezer.

- comments: observed no hot running water on premises at plumbing fixtures (hand sink and 3- compartment sink). Must provide and maintain. (priority 7-38-030(c)

- comments: must provide a garbage can with lid in toilet room.

- comments: must clean debris and dirt build up from walls throughout premises.
