Cockroach infestation. This is the restaurant’s third closure due to cockroaches in less than a month. It was first shut down Oct. 13-15. When the inspector returned Oct. 29 for a new inspection, the infestation was ongoing and it was closed again, then permitted to reopen the next day. After the operator failed to send proof of additional pest control treatment to the health department, an inspector went back Nov. 5 and shut it down again. (The restaurant was also briefly closed in August because it was unable to wash dishes.) It now faces an administrative hearing.
