Cockroach infestation. The restaurant failed an Oct. 13 inspection and was closed for two days because of cockroaches. The inspector returned Oct. 29 for a mandatory rescore inspection and discovered the infestation was ongoing. This is the restaurant’s third closure in three months — it was briefly closed in August because it was unable to wash dishes — and it now faces an administrative hearing.
