- comments: observed outside garbage area not maintained. Overflowing dumpsters open with trash, and around dumpsters. Instructed outside garbage area must be maintain clean, lids closed. Priority foundation issued.

55. Physical facilities installed, maintained & clean
- comments: observed clutter of unnecessary items stored under stairs leading to 2nd floor storage area. Instructed to remove anything no longer using to prevent a pest harborage area.
