Unclean conditions, cockroaches. The inspector found approximately 15 live cockroaches on the wall and floor under 3 compartment sink and food preparation sink, one dead cockroach on the floor under the three-compartment sink, and a dark brown substance heavily accumulated on the cutting board which the employees were actively using.
