Philly Cheesesteak Hospitalization - Uber Eats Delivery, Pittsburg California

A Philly cheesesteak sandwich On 7/7/21, around 5:30pm, I ordered a Philly cheesesteak sandwich from there through Uber Eats. The order arrived around 6:30pm. By 10pm, I was both vomiting and suffering from constant diarrhea. The symptoms became so serious that I lost consciousness, and my wife called 911. An ambulance transported me to the Kaiser hospital in Antioch, California, where I was diagnosed with food poisoning. I was admitted, and spent 3 days in the hospital being treated. I was finally released today, but continue to suffer from the symptoms.
