2. City of chicago food service sanitation certificate
- comments: 2-101. 11 observed no city of chicago food service sanitation manager on site nor a valid certificate posted while time/temperature control for safety foods (chicken) are being prepared handled and served to the public. Instructed to always have a city of chicago food service sanitation manager on site at all times and a valid certificate posted while open and operating. Priority foundation 7-38-012.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: 2-102. 14(o) observed no employee health policy on site. Instructed to provide. Priority foundation 7-38-010.

5. Procedures for responding to vomiting and diarrheal events
- comments: 2-501. 11 observed no written procedure nor adequate supplies on site for responding to vomit and diarrhea events that can potentially occur inside of the facility. Instructed to provide. Priority foundation 7-38-005.

- comments: 6-301. 11 observed no hand washing cleanser available at the rear hand washing sink located by the 3 compartment sink. Employee provided hand washing cleanser during the inspection. Instructed to always maintain hand washing cleanser at all times. Priority foundation 7-38-030(c).

- comments: 6-301. 14 observed hand washing signs are needed at the hand washing sink in the toilet room and at a hand washing sink in the food prep area. Instructed to provide.

- comments: 4-602. 11(b-e) observed a slite pinkish substance inside of the ice machine located in the food prep area. Instructed to clean.

- comments: 5-103. 12 observed cold water shut off from the bottom at the hand washing sink in the food prep area. Employee turned the cold water on and the cold water was functioning properly after the employee turn the cold water on at the bottom of the sink during the inspection. Instructed to maintain cold water at all sinks at all times. Priority violation 7-38-030(c).

- comments: 5-204. 12 unable to locate the backflow prevention devices on the ice machines located in the food prep area and the customer dining area. Instructed to locate.
