- comments: observed person in charge no valid proof of food safety management training during time of inspection. Must provide.
2. City of chicago food service sanitation certificate
- comments: observed facility food handlers preparing and handling time and temperature control for safety (tcs) foods (chicken, fish, beef) without a city of chicago food service manager certificate holder on site. Informed person in charge of the original city of chicago food service certificate must remain posted on site, and a certified manager shall be on site during tcs food prep, cooking, serving, and operating hours. Priority foundation violation #7-38-012. Citation issued.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010. Citation issued
5. Procedures for responding to vomiting and diarrheal events
- comments: observed facility no clean-up supplies and no written procedure for responding to vomiting or diarrheal events. Instructed facility to provide completed cleanup procedure to include sanitizer concentration levels and a list of supplies to clean bodily fluid spills on site. (refer to requirements listed above) priority foundation violation #7-38-005. Citation issued.
25. Consumer advisory provided for raw/undercooked food
- comments: observed no consumer advisory with disclosure/reminder presented on menu for all animal food products that can be ordered raw or undercooked (ex, burger and chop steak). Instructed to provide consumer advisory on all menus. (refer to cdph consumer advisory handout) priority foundation 7-38-005. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed 1 live rat in utility/furnace closet next to pool table and approx. 12 droppings in basement north side crawl space ledge. Additional pest control service is needed to eliminate the pest activity. Priority foundation #7-38-020(a). Citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: noted beer stored in outside shed. Instructed to store indoors in liquor storage room where it is proctected
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed excessive dust/ flour on 2 tier rack in kitchen. Instructed to clean and maintain.

- comments: noted all cutting boards in kitchen with deep stains and cuts. Instructed to restore to a smooth and easy to clean surface by deep clean, sanitize, sand and reseal or replace and maintain.

- comments: observed no chemical test kit for chlorine sanitizer used at 3 compartment sinks. An informed person in charge of a test kit that accurately measures the concentration in mg/l of sanitizing solution must be provided. Priority foundation violation #7-38-005. Citation issued.

- comments: noted heavy grease deposits on bottom compartment of fryer and all cooking equipment. Instructed to deep clean and maintain.

- comments: noted no covered receptacle in west womens washroom. Instructed to provide and maintain.

- comments: noted west washrooms not completely enclosed. Washrooms need tight fitting self-closing doors. Instructed to install.
55. Physical facilities installed, maintained & clean
- comments: noted brooms, dust pans, tiles and other items stored on the floor in southwest utility closet/furnace room. Also noted clutter in basement areas and unused walk-in cooler. Large equipment (stove and smoker) at outside patio. Instructed to elevate items on proper shelving that is non-porous, smooth and easy to clean and a minimum of 6 inches off the ground. Remove all unnecessary and unused items to prevent pest harborage.
57. All food employees have food handler training
- comments: observed no food handler training certificates on site. Instructed to provide for current employees.
