- comments: 2-101. 11 observed no city of chicago food service sanitation manager on site while tcs foods (eggs, milk) are being prepared handled and served to the public. Instructed a city of chicago food service sanitation manager must be on site at all times the facility is open and operating. Priority foundation. Citation issued 7-38-012.
38. Insects, rodents, & animals not present
- comments: no updated logbook on site at time of inspection. Outdated pest control license dated 12/21/24 last service receipt 7/16/25 instructed pic to have logbook up to date with all information. Priority foundation 7-38-020(c) citation issued
55. Physical facilities installed, maintained & clean
- comments: floors in corners of prep area and storage area in need of cleaning to remove all build up. Instructed to detail clean and maintain
56. Adequate ventilation & lighting; designated areas used
- comments: no hood vent on site for smoke and smells of food products. Instructed to eliminate all smoke and maintain.
