- comments: observed no certified food manager on the premises while tcs foods are being prepared and served. Management instructed that a certified food manager must be on premises at all times while tcs foods are being prepared or served. Priority foundation violation 7-38-012. Citation issued.

- comments: observed no signed employee health policy on the premises. Management instructed to provide a signed employee health policy for each food handling employee. Priority foundation violation 7-38-010. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand soap at the exposed hand washing sink at the basement food prep area behind the dishwashing area. Management instructed to provide hand soap at all hand washing sinks at all times. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed no paper towels at the exposed hand washing sinks at the middle bar in the 1st floor dining room, at the basement dishwashing area and in the basement food prep area behind the dishwashing area. Management instructed to provide paper towels or hand drying devices at all exposed hand washing sinks at all times. Priority foundation violation 7-38-030(c). See violation #10 above for citation.
38. Insects, rodents, & animals not present
- comments: observed approx. 20 live small flying insects on the walls and ceiling of the basement food prep and dishwashing areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.
52. Sewage & waste water properly disposed
- comments: observed drain line hoses from the soda gun nozzles emptying into the exposed hand washing sink at the 1st floor bar. Management instructed to reroute the drain lines to a floor drain or dump sink.
55. Physical facilities installed, maintained & clean
- comments: clean the floor in the mezzanine demo prep kitchen spaces.
