Mushroom & Zucchini Illness

I ordered grilled chicken teriyaki (which was fine), with a side of mushrooms and zucchini. The mushrooms & zucchini were the culprits! I ended up with severe diarrhea, nausea, vomiting, fever of 101.6, and colitis!  | Symptoms: Diarrhea, Fever, Nausea, Vomiting
