comments: 3-302. 12 observed 3-5 dry food (flour, white flour, suagar, lemon pepper) storage containers located at the front slushy station/ service area and the rear dry food storage area not properly labeled insturcted to label all dry food storage containers and maintain.

- comments: 3-307. 11 observed scoops, ladels, and metal raminkins improperly stored in all the dry food storage bins on site with the handles making contact with the dry food ingredients. Instructed to properly store all equipment in the storage containers and maintain.

- comments: 4-101. 19 observed raw wood floor storage shelf with wheels in the walk-in cooler with a build up of dirt. Juice was stored on the said shelf. Instructed to seal raw wood, clean, and maintain said equipment.

- comments: 6-501. 13 observed the floors under the shelving units throughout the facility with food (gyro meat), debris, and dirt. Instructed to clean under all shelving units and maintain.

- comments: previous priority foundation violations from report #2572381 03/10/2023 not corrected: #47-observed broken gasket seal on the walk in cooler in the food prep area. Instructed to repair and maintain. #53-observed no covered receptacle in employee toilet room. Instructed to provide and maintain. #55-observed equipment not being used in rear storage/prep area. Instructed to remove all clutter as a means of preventing harboring of pest and insects. Priority 7-42-090, citation issued.
