- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 125 or more mice droppings scattered on floor under liquor pallets,on cardboard at chip display units,along window sills at front window,along wall at ice freezer,behind hot water tank,on bricks at motor in basement and scattered in between articles on floor in same area. Also. Appx. 15 or more live roaches crawling on floor and walls in basement. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 4/23/20. (priority foundation 7-38-020(a)

- comments: observed an appx. '3/4-2' gap along bottom and side of front entry door. Must make door tight fitting.

49. Non-food/food contact surfaces clean
- comments: must clean dust build up from shelving units on sales floor ,all liquor storage shelves,walk in cooler-fan guard covers,drink sleeves & shelving units and ice build up on compressor units. Must defrost and clean chest freezer.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must provide a garbage can with lid in toilet room.

- comments: must clean dirt build up from walls and ceilings in walk in cooler,rear liquor storage areas.

- comments: must repair or replace gaps in between wall and front window,damaged floor tiles throughout liquor storage area and along front windows.

62. Compliance with clean indoor air ordinance
- comments: observed appx. 40 or more cigarette butts on counter top in rear area next to an ashtray and cigarette lighter,on floor scattered under,around and near pallets throughout and on basement floor. Another ash tray in toilet room. Instructed manager there is no smoking on premises only 15 feet away from entrance of premises. (7-32-020(c). Must remove cigarette butts and ashtrays from premises.
