- comments: observed pic doesn't have a food managers certificate. Must provide and maintain.

2. City of chicago food service sanitation certificate
- comments: observed no food manager on duty nor a food managers certificate posted to view while tcs ready to eat foods are being prepared,handled and served such as (italian beef,cooked ground beef,cheese sauce)must provide certificate and be on site at all times. (priority foundation 7-38-012)

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)

- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

- comments: observed no hand washing sink in rear dish washing area. Must provide a hand sink with hot and cold running water and maintain. (priority foundation 7-38-030(c)

- comments: observed evidence of 2- live roaches crawling inside of display unit where hot holding unit is located on top of. Also appx. 30 or more mice droppings on floor and on shelving units in rear storage area and office area. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 5/8/20. (priority foundation 7-38-020(a)
