> - OBSERVED TONGS AND SERVING UTENSILS STORED DIRECTLY ON SOME FOOD WITH HANDLES IN CONTACT WITH THE FOOD. MUST REMOVE AND STORE ALL UTENSILS WITH THE HANDLES UP AND AWAY FROM DIRECT CONTACT WITH FOOD, IN-BETWEEN USAGE. -
> - OBSERVED PRESET TABLEWARE IN THE DINING ROOM, WITH UNCOVERED & EXPOSED FOOD CONTACT SURFACES. MUST COVER ALL FOOD CONTACT SURFACES OF PRESET TABLEWARE TO PREVENT POTENTIAL EXPOSURE TO CONTAMINATION. -

> - UNABLE TO OBSERVE THE BACKFLOW PREVENTION DEVICE FOR THE MOP SINK. MUST BE ABLE TO DEMONSTRATE THE LOCATION OF THE BACKFLOW PREVENTION DEVICE FOR VISIBLE INSPECTION. -
> - OBSERVED SLOW DRAINING HANDWASH SINKS THROUGHOUT FACILITY. MUST REPAIR AND MAINTAIN. -

MUST REMOVE ALL WASTE WATER FROM FLOOR, SANITIZE AREA, AND REPAIR PLUMBING TO PROVIDE ALL WASTE WATER TO BE PROPERLY CONVEYED AT ALL TIMES. PRIORITY VIOLATION
> - MUST PROVIDE THE DOORS OF THE TOILET ROOMS TO BE SELF CLOSING. -
> - MUST REMOVE ALL UNUSED EQUIPMENT AND ARTICLES(EX: OLD COOLER, CEMENT BRICKS) FROM THE OUTSIDE DUMPSTER AREA TO PREVENT POTENTIAL SOURCES OF PEST HABRBORRAGE. -
> - OBSERVED AN ACCUMULATION OF DIRT/DUST AND FOOD DEBRIS BUILDUP ON THE FOLLOWING EXHAUST VENTILATIONS: BOTH TOILET ROOMS VENT AT REAR OF DINING ROOM KITCHEN HOOD MUST REMOVE DEBRIS, DETAIL CLEAN AND MAINTAIN ALL AREAS. -
> - OBSERVED THE LIGHT BULB IN THE REACH IN COOLER IS BURNT OUT. MUST REPLACE AND MAINTAIN. -
> - OBSERVED A NEW EMPLOYEE ON SITE DOES NOT HAVE A FOOD HANDLER CERTIFICATE. ALL EMPLOYEES MUST OBTAIN A FOOD HANDLER CERTIFICATE WITHIN THE FIRST 30 DAYS OF EMPLOYMENT. -
> - OBSERVED NO CERTIFIED MANAGERS ON SITE, AT TIME OF INSPECTION. MUST PROVIDE ALLERGEN AWARENESS TRAINING FOR ALL CERTIFIED MANAGERS AND MAINTAIN CERTIFICATES ON SITE. -
