A sanitary permit was revoked for Benii Japanese Restaurant in Upper Tumon after inspectors from the Department of Public Health and Social Services Division of Environmental Health found multiple health code and sanitation violations.Based on the food establishment inspection report, inspectors noted various violations that included:
Employees were not aware of the employee health policy.
Employees were observed washing their hands in the food preparation sink, or washing their hands without soap.
Multiple food items were left uncovered on table tops and inside chillers.
Utensils were observed being rinsed with water and not washed with soap or sanitized.
Multiple potentially hazardous goods did not meet required proper cold holding temperatures.
Food products and buckets of sauces were placed directly on the floor of the walk-in chiller.
Inspectors also noted the grease trap was not adequately maintained and wastewater from hand-wash sinks and ware washing sinks were draining directly to the floor.
