An inspector ordered that Samurai Japanese Steakhouse and Sushi Bar be temporarily shut down on Aug. 2 after live insects were found in the establishment.
An inspector observed nine live roaches.
Observed approximately 100 insect larvae under a dishwashing machine.
Sticky tape for catching flies was hanging... See More over a food preparation area.
Raw salmon was stored over pasta in a walk-in freezer. The pasta was relocated.
Chemicals were stored with food on the cook line.
Medicine was stored with food on the cook line. An employee moved the medicine.
A bucket of dried shrimp was stored on the floor of a walk-in freezer. A manager shelved it.
A cutting board on the cook line had cut marks and was no longer cleanable.
There was grease accumulated under cooking equipment, on walls and on the cook line.
Raw shrimp was thawing at room temperature. The inspector educated a manager regarding proper thawing techniques, and the shrimp was moved to a reach-in cooler for thawing.
Cut vegetables were stored in reused boxes in a walk-in cooler. Corrective action was taken.
Surfaces in a walk-in cooler were soiled, according to an inspector.
There was an encrusted material on a can opener blade. An employee cleaned it.
The restaurant passed a follow-up inspection on Aug. 3 and was allowed to reopen.

“Absolutely disgusting food and packaging. I ordered the Yum Nua and Mongolia Chicken from GrubHub. Both of the Styrofoam containers were absolutely destroyed and leaking. I'm not sure if this is the restaurants fault or the delivery drivers.
The amount of liquid inside of both containers was astonishing. The bottom... See More third of the container was a pool of liquids.
The Yum Nua was heavily over-seasoned to the point it was almost inedible, not to mention the fact that a salad was turned into a messy soup.
The Mongolian chicken was overcooked and also drowned in some type of liquid.
The most disgusting part of the entire order was the OVERWHELMING SMELL OF URINE. I am not exaggerating when I say it smelled of urine. I've eaten my fair share of authentic Thai, Korean, Chinese, Japanese, and Indian food. When I said authentic, I mean authentic. I'm not basing my review off of previous delivery food. Even the most fermented foods that I've eaten smelled absolutely nothing like this. I am not even slightly joking when I say it wreaked of urine. This is not me mistaking cilantro or some other spice for the smell of urine. I'm tempted to get it tested to see if someone at the restaurant has committed a felony.
I would not have even attempted eating the Yum Nua if I had smelled the Mongolian Chicken first. I set the Mongolian chicken in the sink because it was leaking the soupy mess that was inside.

I wish I would've realized the food might have been contaminated before I ate it, but now only time will tell if I get food poisoning or something worse. I researched why it might smell the way it did and discovered that it could potentially be a packaging method used for prepackaged Thai food, but with the amount of liquid in the dishes that should barely have any... I'm slightly worried that poor food handling is the culprit.

Eat here at your own risk. The entire experience was disgusting and I will absolutely never order from here again.

That is only way I could get out of hospital I got back to states and saw an infection disease specialist it took about 3 months before I could eat without the knawing feeling in my stomach and to have endurance to exercise again. This left me concerned so I ran the remaining cheesecake through a strainer and found a large piece of plastic sheeting as well! I have photos to share as well of the large plastic piece. I was contacted by the risk Manager and was asked to send the items to her so they could be investigated. I mail part of them to her and still have some as well. I was told the large plastic piece was a plastic collar used to assemble the cheesecake. I have not been given an answer as to what the small hard plastic pieces are.
