“I ordered through drive through 2 plates one for my husband and one for me. My plate had fried rice, noodles, sesame chicken, and broccoli beef with a coke. The cashier told me to pull over to the side while they pre my meal so i did, after waiting for 8 minutes she came outside with my food. Once i... got my food i pulled over to eat it and i ate everything on my plate but as soon as i went home i had a bloated feeling in my stomache when all of a sudden i started throwing up everything i ate, when i felt like i was done i took a shower and went to sleep, i woke up out of my sleep 3 times to throw up. The next day i woke up doing diarriah on myself nonstop. I took some pepto, now i just feel weak and i can’t eat anything without throwing up. ”

“Purchase food on the 23rd of February, ordered butter chicken, chili chicken, eggplant, chicken bryiani, and chicken and shrimp noodles.
Symptoms began next day in the morning, heavy dirhea, weakness, for both my wife and I, symptoms continued into the following day but milder. ”

High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed chef going outside to throw away garbage and cleaning outside, returned to food preparation area and continued to cook and plate food.
High Priority - Live,... small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 2 live flies in kitchen area.
High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Observed raw frozen pork over cooked noodles inside walk in cooler.
High Priority - Roach activity present as evidenced by live roaches found. Observed approximately 3 live roaches under three-compartment sink; 3 live roaches crawling on floor under prep table in kitchen, 1 live roach on kitchen wall, and 1 live roach at front counter area on floor.
High Priority - Stop Sale issued due to adulteration of food product. Soil residue in food storage containers. Observed operator refill sugar container while dead roaches lie in sugar container. Instructed operator to discard and wash rinse and sanitize container.
High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed noodles (65°F - Cold Holding) holding at room temperature at prep table in kitchen. Operator relocated to reach in cooler. **Corrected On-Site**
High Priority - Toxic substance/chemical improperly stored. Observed hydrogen peroxide and pain medicines stored above food preparation area across from cook line.

“Ate a grilled chicken sandwich there yesterday. Came home fine but after about 6 hours I started feeling nauseous with excruciating cramps and then horrible diarrhea. “Today, Me and my family postmated it. I ate a burrito bowl with egg whites from here I ate the bowl and immediately my stomach hurt after and the next day(03/01/21) my liver and stomach was hurting and i woke up nauseous and later i ended up having diarrhea. I believe it was most likely from the ham... and also my mother had the ham and she said it tasted spoiled and she also was feeling nauseous and had stomach pains. So 2 people. ”

“A multistate outbreak of Listeria monocytogenes infections potentially linked to Hispanic-style fresh and soft cheeses is currently being investigated by the FDA, CDC, and state & local health agencies. Hispanic-style fresh and soft cheeses are currently the leading hypotheses for the cause of the L... isteria infections.
As of now, there are reports of seven people infected with the outbreak strain of Listeria monocytogenes in Connecticut, Maryland, New York, and Virginia. Six of the seven ill people are Hispanic. Of the four people interviewed, three reported eating at least one type of Hispanic-style fresh and soft cheeses, and all three reported eating queso fresco.

“I was in Vegas for my birthday and had a great week. At the end of the week we went to my favorite restaurant and I had a double double, fries, coke, and milkshake. My husband and I ate the exact same meals all day except I got lettuce on my burger and he had water and I had a coke. I'm pretty sure... the lettuce was contaminated or it was my drink. Just a few hours after eating I got all the symptoms mentioned:( It was so tragic to happen at my favorite restaurant:( ”
