In some individuals the consumption of undisclosed allergens could cause allergic reactions that in some cases could be life-threatening. The recall was necessitated when Fresh Express learned that, during a single production run, incorrect condiment packets were placed into Southwest Chopped Kit bags and, as a result, the allergens wheat, soy, cashews, and coconut are not properly declared.

Recalled Product Details:
- Fresh Express Southwest Chopped Kit (11.5-oz. bag). UPC Code: 0 71279 30602 5 (located on the reverse side of the bag by the bar code). Product Codes: G163B10A / G163B10B. Use-by: June 29 (located in the upper right hand corner on the front of the bag).

Only the Southwest Chopped Kit displaying the identified product codes, UPC code and use-by date are subject to the recall. All other Fresh Express Southwest Chopped Kits are properly labeled, and no other Fresh Express products are included in the recall. The recalled product was distributed between June 12 and June 18 in the states of Alabama, Delaware, Florida, Georgia, Kentucky, Maryland, North Carolina, South Carolina, Tennessee, Virginia, and West Virginia. Fresh Express representatives are coordinating with retail stores, instructing them to remove the recalled product from store shelves and inventories.

Fresh Express takes all matters of food safety very seriously, including the issue of allergens. Company procedures and programs stringently follow all mandated regulations and focus on preventive measures designed to minimize potential risks. Express is coordinating with the U.S. Food and Drug Administration and is conducting a full investigation into this isolated incident.

Consumers in possession of the recalled product should discard it. A refund is available where purchased or by contacting the Fresh Express Consumer Response Center toll-free at (800) 242-5472 Monday through Friday during the hours of 7 a.m. to 5 p.m. Eastern time.

There have been no confirmed illnesses reported to date.

The recalled Captain Rusty’s Seafood Dips were distributed in South Florida, Sarasota, FL., Tampa, FL. and Lorida, FL. through distributors and retail markets. The product comes in a 7-ounce, clear plastic container, 16-ounce clear plastic container, and 6 lb white plastic buckets. The lot code is located on the bottom of the container and the expiration date is located on the top label.

The recall was initiated after the Florida Department of Agriculture and Consumer Services (FDACS) determined, during a routine inspection, that the product containing allergens was distributed in packaging that did not identify the presence of fish (mahi, salmon, whiting, pollock, threadfin bream), crustacean shellfish (crab, lobster, shrimp), wheat, eggs, soy or milk. Subsequent investigation indicates the problem was caused by a breakdown in the company’s labeling process. The recalled “Home Style Crab Cakes” were distributed in retail markets throughout Alabama and Florida from 4/28/20 through 5/11/20. Julian date of 212220 is stamped on master cases for distributors and retailers to identify effected cases. This products comes in a 12oz black retail freezer tray containing 4- 3oz crab cakes. The outer labeling says “Johnson Sea Products Home Style Crab Cakes” in blue, white and yellow letters. Please see the exact label attached.

No illnesses have been reported to date in connection with this problem. The recall was initiated after it was discovered that the wheat and soy containing product was distributed in packaging that did not reveal the presence of wheat or soy. Subsequent investigation indicates the problem occurred when an out-dated sample label was used on the Home Style Crab Cakes that contain wheat and soy.

Product was distributed to Coremark Distribution centers that distributed product to 18 stores, Utah state wide. The product labels look as follows: (see below) The description of the products in question are: Roll chicken salad on croissant single item# 43237 Label UPC: 50777445351 and sndw chicken sandwich on white single item# 43559 Label UPC: 50777444804 A wrong ingredient was purchased and used; this ingredient chicken chunks contains soy and was not declared originally in the labels. The code dates “USE THRU” 03/6/2020 – 05/6/2020, and “USE THRU” 03/6/2020 05/2/2020.

There have been no documented illnesses at this time. After an inhouse audit of our ingredients by our Quality Assurance Manager it was discovered that the wrong ingredient had been purchased and used, it was decided to initiate a voluntary recall to ensure that our public would not get further exposure to an erroneous product. People who have an allergy or severe sensitivity to wheat, soy, milk, fish, or eggs run the risk of a serious or life-threatening allergic reaction if they consume this product.

Flounder Stuffed with Seafood was sold to a limited number of consumers between April 1, 2020 and April 16, 2020 at various locations across the Harris County, Texas area. Nearly all customers who purchased the product have been notified directly, however the purchasers of less than 25 cases (75lbs) of the product have not been identified or notified. The product is packaged in a brown cardboard box with the product name "FLOUNDER STUFFED W SEAFOOD FRZN." The Item Number is 7126763. The Pack Dates on the label range from 03/31/20 to 04/15/20. Customers with allergies or sensitivities to wheat, soy, milk, fish, or eggs should discard the product or contact Buckhead Meat and Seafood of Houston to return it for a full refund.

To date, no illnesses related to this product have been reported. The issue was identified by a company employee while evaluating the product. Buckhead Meat and Seafood of Houston is working collaboratively with the U.S. Food and Drug Administration on this recall. Buckhead Meat and Seafood of Houston regrets any inconvenience and concern this recall may cause.

Ghost Pepper Queso includes two allergens, milk and soy, that are not included in Ghost Pepper Salsa, and therefore don’t appear on the ingredient statement of the mislabeled jars. People who have an allergy or severe sensitivity to milk and soy run the risk of serious or life-threatening allergic reaction if they consume these products. While Stonewall Kitchen believes that only a small number of jars were mislabeled out of the 4,812 which were manufactured that day, the company is recalling all of them out of an abundance of caution.  Of note, the mislabeled jars should be very easy to spot: Ghost Pepper Queso product is a bright yellow-orange color, while Ghost Pepper Salsa is dark red. This recall is due to possible undeclared allergens (peanut, wheat, soy and tree nuts) in H-E-B-branded Tuscan Herb Salad Kit, which was manufactured by Dole for H-E-B. The incorrect masterpack (kit with dressing and other toppings) was unintentionally used during the production of the salad. No illnesses or allergic reactions have been reported, to date. However, people who have an allergy to peanut, wheat, soy and tree nuts may have a serious or life-threatening allergic reaction if they consume these products or products containing the allergens listed above.

This recall is for H-E-B-branded Tuscan Herb Chopped Salad Kit that is sold at H-E-B stores across Texas. The product code and best-by date are located on the top right corner of the front of the salad bag. Consumers who have purchased the designated products are instructed not to consume the product and to call the DOLE Consumer Center toll-free at 1-800-356-3111, which is open 24 hours a day, for a refund.

Food safety is the first priority of Dole Fresh Vegetables, Inc., so although we cannot confirm that all packages have the incorrect masterpack, this recall is being initiated in an abundance of caution for the benefit of consumers.
