3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: found no employee health policy/training on site. Instructed facility to establish an appropriate employee health policy/training system and maintain with verifiable documents on site. Priority foundation violation 7-38-010.

5. Procedures for responding to vomiting and diarrheal events
- comments: found no procedure/plan and kit for responding to vomiting and diarrheal events. Instructed facility to develop and maintain a procedure/plan and to maintain any appropriate supplies on site. Priority foundation violation 7-38-005.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed empty bottle of sanitizing solution at low temp dish machine at bar area while glasses were being washed, 0 ppm chlorine. No other full bottle of sanitizer on site(per manager statement). Instructed to provide. Low temp dish machine tagged, 'do not use' priority violation 7-38-005

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must not use milk crates as a means of elevation for stored items.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: noted premises using a high temperature dish washers. Noted no temperature test strips. Instructed to provide and irreversible registering temperature indicator,strips/stickers. Priority foundation violation:7-38-005.

- comments: no chemical test strips on site to check sanitizing levels for the bar three compartment sink and low temp dish machine. Must provide test strips according to sanitizer solution used on premises. Priority foundation violation 7-38-005

- comments: no back flow prevention device provided at mop sink second floor by the conference/class room. Must install so back flow prevention device may be located to be serviced and maintain.

- comments: some debris on floors under shelves included bare area,clean and maintain. Observed a lot stock stored on floor, instructed to elevate items 6' off the floor throughout the premises.
