- comments: observed no clean-up of vomiting and diarrhea procedure and clean-up kit on premises during inspection. Instructed to have said procedure along with required equipment for clean-up. Priority foundation violation. No citation issued. 7-38-005

- comments: noted the bathroom door at the rear area not self closing due to broken closure device. Instructed to repair a self closing device at the door and maintain

- comments: noted heavy clutter throughout establishment on floors inside unused equipment and in storage areas. Instructed to detail clean, organize and remove all unnecessary or unused articles and equipment to prevent hiding places for pest. Must upgrade housing keeping.

59. Previous priority foundation violation corrected
- comments: previous priority foundation violations from report #2373561 06/11/2020 not corrected. Violation #38 observed that pest control log book is missing required information. Missing record of visits, missing copy of service ticket that sets forth pest control idph license number violation #54 observed that facility doesnt have commercial trash receptacle. By statement of owner they using building container but no copy of contract provided. Facility failed to provide required information in timely meaner. Priority violation #7-42-090. No citation issued
