- comments: 5-501. 17 observed no covered receptacles inside of the toilet rooms. Instructed to provide.

- comments: 5-501. 17 observed holes in the walls in various areas of the rear dry storage and in the toilet rooms. Instructed to seal all holes in order to prevent possible pest entry and make all surfaces smooth and easily cleanable.

- comments: 8-404. 13(b:3) observed previous priority foundation violation not corrected from inspection report #2633054 dated 3/25/26; 38 - 6-501. 111 observed evidence of pest activity on site. Observed about 100 rodent droppings scattered in various areas of the rear storage area, under shelves and on top of the bottom shelves in various areas and in the toilet rooms. Instructed to eliminate rodent activity, clean and sanitize affected areas and recommended contacting pest control for a service. Upon re-inspection still observed evidence of pest activity on site.
