- comments: observed raw wood front counter and under pizza window wall line. Instructed manager to seal wood at said locations, wood surfaces must be smooth and easy to clean.

- comments: observed accumulted dust build up on prep area reach in coolers condensers. Must clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed dust/debris on washroom walls and dirt build up inside floor drains. Must clean and maintain washroom walls and interior of floor drains.
56. Adequate ventilation & lighting; designated areas used
- comments: observed prep area ceiling light bulbs not working (outside of walk in cooler). Must replace with working ceiling light bulbs.

“I got a medium pan pizza last night and was fine then i ate the pizza at 9 a i got my food around that time later arounf 11 I started feeling nauseas and having bad diarrhea; I thought somthing else was going on so i put the pizza in... the fridge and woke up the next day feeling okay but i ate some more and got severly sick vomiting and diarrhea and a headache. ”
“I ordered a large pepperoni pizza. I was feeling fine until after I ate the pizza. And early in the morning symptoms started where I was coughing a lot, choking on flem and then started throwing up. I eventually lost my voice for a whole week. This happened 2 times... in One in June and the other in July 2020. I will never go back there. ”

I went to sleep woke up the next day still with diarrhea no vomiting ”
“Sushi I purchased the Spicy Tuna Roll, and California Roll in the Sushi Dept. I was sick about 2 hours later with intense abdominal cramps, diarrhea, and vomiting. I had to go to the ER for IV, Potassium, Cat Scans, and Blood Work. They're doing little to nothing about this.... The manager on site did NOTHING.
Their corporate said oh, well we'll get back to you to see what we could do. They said it might not even be food poisoning and it could have just been a virus I had and the sushi brought it out of me. I was APALLED!!!! I asked if she was a doctor, she said no. I said so then let's stick to the doctors who actually treated me. She told me doctors blame a lot on food poisoning when in fact it is not. She also mentioned that since they hadn't had an illness from Sushi since 2016 she doesnt think the Sushi made me sick.
I'm unsure why they are rated #1 in the US. I will NEVER shop there ever again!!!! ”
