The Habit Burger & Grill 4210 E 4th St, Ontario, CA 91764 Ordered take out on 12/1/24 in the 4 PM hour. Wife and I had the same burger and fries order but she asked me to get her some jalapeños on the side. The jalapeños are out in a self service station to the left of main counter. Filled up a small plastic cup with lid with jalapeños that she put on her burger. After eating around 4:30 PM my wife woke up at ~ 2:15 AM with diarrhea and vomiting. This has continued throughout the day into the afternoon now on the following day. The only different food she and I ate were the jalapeños she asked for. Jalapeños which are sitting out uncooked and harboring bacteria or other pathogens. Tried calling the restaurant when the store is open around noon and their automated answering directed me to the Contact Us section of their website to contact them and have it directed to the appropriate channels. That webpage gives a 404 Error: Page Not Found. (909) 850-3030 order.habitburger.com/contact-us/ I called the county health department to report it and was directed to the EHS website to submit a complaint which I have already done (case #CO0093783).
