- comments: observed no handwashing signage in restrooms on floors 1-3. Instructed person in charge to provide handwashing signage at all handwashing sinks throughout facility.

- comments: observed approximately 60 mouse droppings scattered in the storage in the lunchroom area and in the computer mdf room. Instructed person in charge to clean and sanitize areas and inform pest control service. Priority foundation #7-38-020(a).

- comments: observed no covered receptacle in women's restrooms on the third floor. Instructed person in charge to provide and maintain covered receptacles in restrooms for female use.
