- comments: no employee health policy. Must provide. Priority foundation violation 7-38-012(a) no citation issued.

6. Proper eating, tasting, drinking, or tobacco use
- comments: observed that employees are eating in food prep area. Must provide designated area for employees to eat away from prep area

10. Adequate handwashing sinks properly supplied and accessible
- comments: a sign or poster that notifies food employees to wash their hands not provided on exposed hand washing sinks. Signs shall be provided at all handshaking sinks used by food employees

- comments: noted no working thermometers inside refrigeration unit conspicuously posted to monitor the ambient temperature of equipment. Instructed to equip all refrigeration units with accurate and working thermometers

- comments: noted assorted food items inside coolers and refrigeration units food stored inside grocery bags. Instructed to properly store all food items inside approved food grade containers

- comments: observed large meat cutting and meat grinding equipment to be bigger than 3 compartment sink and cant be submerged in wash water in sink. Must provide cleaning procedure for each separate piece of food-contact equipment with description how to be cleaned in place and/or must submit the procedures to cdph at food@cityofchicago. Org for approval or present procedure to department representative prior inspection for on site review and approval. No citation issued.

- comments: noted no sanitizer test strip kit on site at the time of inspection. Instructed to have a test strip kit on site at all times to properly monitor the strength of the sanitizer. Priority foundation. 7-38-005 no citation issued

- comments: observed no back flow prevention on food grade hose used for washing large equipment.

- comments: must repair or replace stained and damaged ceiling tiles in food prep area, rear storage and bathroom area

- comments: noted employee belongings on shelves at the dry storage area and prep area with food items. Instructed to provide a designated area for employees to store their belongings.
