- comments: observed no valid city of chicago certified manager on duty while tcs foods are being prepared and handled. Instructed manager a city certified manager must be on duty at all times. Priority foundation citation #7-38-012
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: ** premises has no employee health policy. Instructed the manager to comply with the employee health policy or citations will follow. Priority foundation violation #7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: **premises has no clean up procedures or supplies for responding to vomiting and/or diarrhea events. Instructed manager to comply with requirement or citations will follow. Priority foundation violation #7-38-005.
37. Food properly labeled; original container
- comments: instructed manager to label all pre prep food items with dates (fifo).
38. Insects, rodents, & animals not present
- comments: observed 20 mice droppings scattered on the liquor display shelving and bar counter top, 20 mice droppings on the floor in the corner in the kitchen. Instructed manager to remove droppings and detail clean affected areas. Instructed manager to contact pest control for service. Priority foundation citation issue #7-38-020(a).

- comments: observer the rear exit door with a 1/4' gap at the bottom. Also noted an opening (mail slot door mising)in the front door. Instructed manager to seal and/or repair openings to prevent further pest issues.

- comments: instructed manager to detail clean all counter tops, prep table and storage and display she
55. Physical facilities installed, maintained & clean
- comments: floors need detail cleaning through out: behind the bar, in the kitchen, rear and front dining areas.
