- comments: 6-301. 14 observed no hand washing sign at the hand washing sink in the food prep area or in the toilet rooms. Instructed to provide.

38. Insects, rodents, & animals not present
- comments: 6-202. 13 observed a rear door with a one inch gap at the bottom. Instructed to make said door tight fitting and rodent proofed.

55. Physical facilities installed, maintained & clean
- comments: 6-501. 12 observed accumulated dirt debris on the floors in both utility closets. Instructed to clean and sanitize.
