- comments: observed evidence of pest activity, noted approximately 30 rodent droppings in cafeteria office between desk and east wall, and in southeast corner filing cabinet and along base boards of south wall. Instructed to remove and sanitize affected areas.

- comments: noted debris and piling leaves surrounding outdoor recycling enclosure. Instructed to remove and maintain.
55. Physical facilities installed, maintained & clean
- comments: noted clutter in all pre-k building classrooms where students consume meals. Instructed to organize and remove all unnecessary items. All kept items shall be stored on proper shelving that is non-porous, smooth and easy to clean and a minimum of 6 feet off the ground to ensure proper cleaning and monitoring for pest activity.
