Permit is suspended due to cockroaches infestation as evidenced by 15+ live cockroaches crawling on top of the clean food trays stored on top of the drain board in the warewashing sink, next to meat display case..
You are hereby directed to discontinue operating your food facility at once.

Public Health Permit is suspended due to cockroaches infestation. Facility is not approved to operate.

At once, discontinue all food operation (sales, food handling, food preparation, etc.) until such time as the permit has been re-instated and facility has been approved to operate by this Department. Failure to comply will result in further legal action(s).

Pursuant to Los Angeles County Code 8.)$.945 & California Health & Safety code section 114444, your Public health permit

You are hereby directed by this Department to appear for a Compliance Review regarding this matter.
