- comments: observed that employees wore watches on hands while working with exposed food. Must not wore unproved jewelry on hands while working with exposed food

- comments: must clean food debris build up from counter tops,wheel bases of all equipment.

54. Garbage & refuse properly disposed; facilities maintained
- comments: noted outside garbage dumpster overloaded and preventing proper lid closure. Must keep dumpsters covered at all time. Priority foundation violation 7-38-020(b) no citation issued

55. Physical facilities installed, maintained & clean
- comments: noted the following:floors throughout premise especially along wall base and underneath equipment with dirt and food debris, instructed to detail clean and sanitize. Must upgrade housekeeping.
