In 2019, this location had its food service license suspended by the Middletown City Board of Health. Between July 2017 and March 2019, there were 21 inspections and 296 violations with 36 listed as critical violations. It remained closed for around 2 years, until Sep 2021 it reopened for drive through only. It now appears that it has been shut down again.
