- comments: found hand washing sink in mens wash room not maintained not in operation at this time. Hand washing sink must be operational and maintain so that it is accessible at all times for hand washing. Instructed to use other wash room for hand washing and to maintain. Priority foundation violation 7-38-030 (c),citation issued. Manager immediately called for maintenance service. (cos).

- comments: observed black slimy substances on interior water components of the ice maker,dripping into ice 'ice is used for human consumption'. Management was instructed to turn off machine, remove ice and have interior of ice machine cleaned and sanitized and to maintain. Priority foundation violation 7-38-005,citation issued. Manager immediately called for maintenance service. (cos).

- comments: observed a torn door gasket on left door of the 3 door food prep cooler showed manager. Management instructed to replace the torn door gasket.
