Inspectors found several sanitation, pest control, and maintenance problems during the inspection. Significant food safety violations included a previously cited pest issue that had not been corrected: inspectors noted rat droppings in multiple areas and live roaches in the prep and dining areas. A citation was issued for this priority violation. Inspectors also found dead roaches in the rear prep area, a dead mouse in the basement, and gaps under the front and rear doors that could allow pests to enter.Other notable findings included raw sewage on the basement floor and containers of sewage in the basement, uncovered bulk food containers, unlabeled bulk containers, soiled wiping cloths left on sinks and prep tables, and bowls and cups being used as food scoops. Food handlers were not wearing hair restraints, single-use items were not protected from contamination, and cooler door gaskets were damaged.
Inspectors also observed food debris and spills on equipment and surfaces, clutter and dirty dishes in the dishwashing area, unused items stored in prep and basement areas, damaged walls and floors, openings near outlets and the ceiling, and water leaking from the ceiling. The previous inspection summary was not posted for customers to see, and a citation was issued for this priority foundation violation.
