Flight Meal Causes Illness - United Airlines, Virginia

On flight 291 on January 1, 2016. Was on a flight from Dulles to Sacramento sitting in first class. Had the chicken dish. It includes a salad with mushrooms. This was a 5+ hour flight. By hour three i was very sick and bloated with nausea. I told the stewardess and had the sickness bag ready. Made it to the hotel and spent the night vomiting with chills and diarrhea.
