- comments: found no city of chicago food sanitation certificate nor manager on site while preparing/serving tcs foods. Instructed to have a valid certificate and the certificate holder on site while preparing/serving tcs foods. Priority foundation violation 7-38-012 citation issued

- comments: observed cook handling ready to eat and raw foods all while wearing the same gloves. Instructed to change gloves and properly wash hands when multi-tasking. Priority violation 7-38-010 citation issued

- comments: found tcs and ready to eat foods stored inside the walk in cooler not properly labeled. Instructed to properly date/label items held for more than 24 hours. Priority foundation violation 7-38-005 no citation issued
