Report date: Apr 30, 20205. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation -7-38-005)
38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 50 or more mice droppings along wall bases and in between massive amount of articles and clutter in rear storage area. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 4/30/20. (priority foundation 7-38-020(a)
49. Non-food/food contact surfaces clean
- comments: must clean dust and debris build up from all reach in coolers and freezers,display shelves throughout premises.
51. Plumbing installed; proper backflow devices
- comments: must unclog mop sink. Observed standing dirty water inside of basin.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must provide a garbage can with lid in women's toilet room.

- comments: must repair or replace stained ceiling tiles throughout premises,holes in walls and ceilings in men's toilet room,damaged floor tiles at entry doors in vestibule.
