- comments: wooden cutting board in the kitchen in poor repair. Observed board with excessive dirt, grease and deep cuts into the wood. Instructed to replace or resurface.

- comments: must not use tin foil as liner for shelving on the cooks line. Must be smooth and cleanable.

- comments: hot and cold water handles switched on the bar hand washing sink. Must repair/switch so the hot handle dispenses hot water and cold handle dispenses cold.

- comments: floor behind and around the bar box beer cooler with dirt and debris. Must clean and maintain.

- comments: instructed the new city of chicago certified food manager, junior garcia caceres, must obtain the allergen training and provide certificate or proof of training.

- comments: previous core violations from #2282259 on 3-28-19 not corrected. #47- observed raw wood shelves above dish wash 3-compartment sink. Instructed manager must seal said raw wood shelves, shelves surfaces must be smooth and easy to clean. #51- back flow prevention devices not located on ice machine water line. Instructed manager must provide and make visible back flow prevention device for ice machine. #57- no food handlers training certificates on premises. Instructed manager food handlers must have certificates with names of employees on premises. Priority foundation violation 7-42-090 citation issued.
