- comments: observed the rear exit door weather stripping worn at the bottom allowing possible entry point for pests. Must repair and maintain

- comments: observed 15 mice droppings scattered on the storage shelving in the kitchen. Noted 20 mice droppings scattered along the wall base in a vacant class room/storage area behind computers. Noted 5 droppings on the floor in the sunshine room with a hole in the wall. Must detail clean and sanitize all affected area and call pest control for service. Prioirty foundation citation issued #7-38-020(a).

- comments: must detail clean and sanitize food storage shelving in the kitchen to remove droppings

- comments: floors through out need cleaning through along the wall base and in corner to remove debris and droppings.

- comments: observed a few holes and openings in the floor base in the class rooms (carebear, sunshine and caterpillar). Instructed manager to seal all opening where ever needed to prevent further entry points for pests
