- comments: observed 40 mice droppings scattered along the wall base behind the ice machine instructed manager to remove and detail clean area. Last service date was 5-18-20. Priority foundation violation #7-38-020(a).

- comments: must clean the interior of the reach in cooler to remove dust from the fan cover.
51. Plumbing installed; proper backflow devices
- comments: observed leaks at the faucets at the 3 comp sink at the front bar and in the kitchen. Also the last sink at the bar has a slow drain.
55. Physical facilities installed, maintained & clean
- comments: noted multiple hole and openings into the walls through out premises. Instructed manager to seal where ever needed to prevent entry points and hiding place for rodent.

- comments: instructed manager to detail clean all floors through out the prep area and stoarge rooms to remove debris. Also keep floors as dry as possible in the kitchen to prevent breeding conditions.

- comments: instructed manager to organize as much as possible to prevent harborage in storage areas. Must provide better inspection aisles in storage areas for cleaning and pest monitoring.
