Report date: Oct 30, 20202. City of chicago food service sanitation certificate
- comments: observed no city of chicago food sanitation manager on site while tcs foods (pork, chicken, etc. ) are being prepped. Observed no city of chicago food sanitation manager certificate on site. Instructed must have a city of chicago food sanitation manager on site at all times when tcs foods are being prepped, cooked and/or served. Must post chicago food sanitation certificate in plain view of customers at all times. Priority foundation violation 7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed only hand sink not maintained. Found no cold water available at handsink, only hot water. When cold water handle turned, no water comes out of faucet. Instructed must repair hand sink to provide hot and cold water. Priority violation 7-38-030(c). Citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: must install splash guard on hand washing sink to prevent contamination (to items inside 1 compartment sink).
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed raw wood used on walls near potato cutting machine. Must seal or paint to make non-porous, smooth and easily cleanable.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed 3 compartment sink not maintained. Found liquid leaking from pipes under left and middle basins of 3 compartment sink. Instructed must repair to make 3 compartment sink function properly. Priority foundation violation 7-38-025. Citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed excessive clutter in office/storage room. Must elevate all items at least 6 inches above the floor. Must discard any items not in use, and must organize clutter to prevent pest harborage. Must maintain same.
61. Summary report displayed and visible to the public
- comments: observed no summary report posted from inspection # 2268735, dated 2/28/19. Must post most recent inspection summary report in plain view of customers at all times. Priority foundation violation 7-42-010(b). Citation issued.
