- comments: observed black slimy substances on interior water components of the ice maker,dripping into ice "ice is used for human consumption". Management was instructed to turn off machine, remove ice and have interior of ice machine cleaned and sanitized and maintain. Priority foundation violation 7-38-005,citation issued.

- comments: observed gap opening mode of entry at the bottom of rear exit door. Must make door tight fitting all across the bottom to prevent pest entry and to maintain.

- comments: observed rust on the wire storage racks in the walk-in cooler. Management instructed to remove rust and repaint or replace the shelves and to maintain.

- comments: observed washrooms in the premises with garbage receptacles and no lid. Instructed to provide and maintain trash cans with lids at all unisex wash rooms.

- comments: previous core violations #39,#47,#51,#55,#58 not corrected from report #2293403 on 06/12/2019 : #39-all buckets with food and food containers stored on the floor inside the walk in cooler and walk in freezer must be stored 6" off the floor or provide raised shelvings to store food stocks, bucket with food and food containers. Sep 12, 2019 #47-must remove all cardboard being used as a shelve liner in all wire rack shelvings in the prep area in the basement. Sep 12, 2019 #51-backflow prevention device not located on the ice maker and mop sink. Instructed to install to be able to locate, serviced and maintained. Sep 12, 2019 #55-must remove all cardboard on the floor in the prep area in the basement being used as a floor mat. Replace with regular rubber floor mat. #58-all certified food service sanitation manager must obtain an allergen training certificate. Sep 12, 2019 instructed to correct noted violations and maintain by 11/3/2020. Priority foundation violation 7-42-090,citation issued.
