I ate from panera bread on 9/30/2016 @ approximate 330pm. Started with diarrhea at 530 am on 10/1/16 followed by vomiting, fever constantly for 7 hrs which time went to urgent care blood pressure had dropped to 80/60, bun 32. On 10/1 at approc 1.40 pm, ordered foot long ham sub on honey oat bread. Added spinch, lettuce, tomato, pickles, olives, mayo and tzaziki sauce. At 9 pm began throwing up with stomach cramps then by 10 pm had diarrhea, vomiting and fever.  | الأعراض: إسهال, غثيان, حمى, قيء, تشنجات
